Introduction of Service
The Government will launch a four-year Pilot Scheme for setting up district-based multi-disciplinary teams to provide outreach service for the Service Users of private RCHEs to meet their social and rehabilitation needs. Baptist Oi Kwan Social Service is the service operator in New Territories East Cluster.

Referral form and Agreement


 The Scheme aims to

(a) provide a package of Service by social workers, occupational therapists and physiotherapists for the Service Users of private RCHEs to meet their social and rehabilitation needs;

(b) promote the quality of life of the Service Users of private RCHEs through social networking; and

(c) provide speech therapy service by speech therapists for the Service Users of private RCHEs, contract RCHEs(including the DCUs attached to contract RCHEs) as well as self-financing RCHEs and self-financing NHs to address their swallowing difficulties and speech impairment.

 

Service to be provided by us:

For Private RCHEs

(a) On-site visits by social workers, occupational therapists, physiotherapists or speech therapists to the serving RCHEs;

(b) Provision of linkage activities inside or outside the RCHEs in connection to other social service units, volunteer groups, schools, local community organisations and commercial sector, etc. to meet the social needs of the Service Users as well as to promote the care and concern to the elderly in the community;

(c) Clinical assessments on Service Users’ rehabilitation needs for occupational therapy, physiotherapy and speech therapy;

(d) Rehabilitation training programmes including, without limitation, the provision of maintenance exercises, physical training, cognitive training, swallowing improvement exercises, speech training and any other therapeutic activities in the form of individual or group service;

(e) Periodic reviews to monitor the Service Users’ rehabilitation progress; and

(f) Consultations / training programmes to the staff members / carers of the residents of the serving RCHEs; 

 

For contract RCHEs (including the DCUs attached to contract RCHEs) and self-financing RCHEs and self-financing NHs

(a) On-site visits by speech therapists;

(b) Clinical assessments on Service Users’ rehabilitation needs for speech therapy;

(c) Speech therapy including, without limitation, the provision of swallowing improvement exercises, speech training and any other therapeutic activities in the form of individual or group service;

(d) Periodic reviews to monitor the Service Users’ rehabilitation progress on speech therapy; and

(e) Consultations/ training programmes from speech therapists to the staff members/ carers of the serving RCHEs and NHs.
